An EOB is a statement to covered individuals explaining what medical treatments and/or services are paid for on their behalf. Use the interactive FAQ section below to understand yours.

You will be able to view EOBs for yourself and dependents under the age of 18 living at the same address as you.

Yes, each member can have their own account.

If the dependent is at a different address than you then you will not be able to see their EOBs online when logged in under your account. Dependents with an alternate address can choose to receive their EOBs by mail at the alternate address, or they can create an account to see their EOBs online.

EOBs remain accessible via the web for five years.

Yes, you can choose to receive paper or electronic EOBs. To change your preference, please contact us via our online contact form or call AultCare's Customer Service at 330-363-6360 or 1-800-344-8858 and a representative will assist with changing your EOB delivery options.

Notices are sent out once every two weeks. You may or may not receive a notice, depending on if you have had any recent claims activity.

To change your e-mail address, please follow these steps:
1. Log into your account
2. Click on the Account tab
3. Choose Contact Information
4. Update your email address
5. Click Save

Yes, you can receive electronic EOBs but stop receiving the notifications. To change your preference, please contact us via our online contact form or call AultCare’s Customer Service at 330-363-6360 or 1-800-344-8858 and a representative will assist with changing this option.

You will be notified when and how to create an AultCare account .

Your password will be valid for a year after termination so your claim history may still be accessed.

IRS Form 1095-B

AultCare Insurance Company will not be automatically mailing 2023 Form 1095-Bs to members. However, upon request, any applicable members can have their 2023 Form 1095-B.

To receive your 2023 IRS Form 1095-B, submit the request via our CONTACT US on our website or send the request to:
